Hagen Psychiatry, LLC (“The Company”) may receive, collect and store any information you enter on this website (www.hagenpsych.com). Software tools, such as those provided by Google Analytics, may be used to measure and collect session information, including page response times, length of visits to certain pages, page interaction information, and methods used to browse away from the page. This information is recorded in a way not identifiable to you but may include indirect ways in which your identity could be discovered (e.g. via IP address). However, this information is not intended to be, and will not be used by the Company intentionally to identify you.
If you voluntarily provide additional personally identifiable information via the online Contact form (e.g. name, phone number, email, health insurance information, and any other content you choose to enter), all of that information is collected and stored exclusively for use by the Company for the sole purpose of contacting you to discuss your potential involvement with the Company for psychiatric services as defined in the “About My Practice” section of the website. The only exception to this is if you include any statements concerning for your immediate safety, or the safety of others (e.g. mention of suicide, inflicting self-harm, or of harming others) in which case Bryan Hagen, DNP, PMHNP, as the intended recipient of your message, is legally obligated to make reports to law enforcement, emergency medical services, or other relevant parties to ensure the safety of you or of others.
Be aware that Contact form submissions are not guaranteed to be HIPAA-compliant, and that the intent of this form is simply to request direct contact from the Company in order to establish care. The Company assures all Contact form submissions will be treated as private and as Protected Health Information, but cannot assure that private health information shared via this form is secure to the same degree as is required of a HIPAA-compliant medical record.
This website is hosted on the Wix.com platform. Wix.com provides us with the online platform that allows the Company to advertise services to you. Your data may be stored through Wix.com’s data storage, databases and the general Wix.com applications. They store your data on secure servers behind a firewall.
The Company reserves the right to modify this privacy policy at any time, so please review it frequently. Changes and clarifications will take effect immediately upon their posting on the website. If the Company makes material changes to this policy, the Company will notify you here that it has been updated, so that you are aware of what information collected, how it is used, and under what circumstances, if any, it may be used and/or disclosed.
